I think the CCM should be multisectoral to accormodate all sectors, not only health. And sometimes the Health Ministries heading the CCM causes problems. Even the composition of the CCM is influence by societal popularity. This should be stopped.

Received reminder& coverage of the partnership Forun 2008,I am sending discusion e-FORUM 2008 based on GF Secretariat priority, &implementation.Degree of representation,responsive to all stake- holders,community system strengthening ,measures taken capacity, capacity building,& mobilizing resources factors could decide GF- Secretariat proper functioning.Selection of GF Secretariat from PR,MOH,& organisations should be depend upon above factors before evaluation of transparency,level of consecration&eccentricities. -------------------------------------------------------------------- GF Secretariat should take number of measures e.g availability of all ART drugs,proper adherence to the treatment &quality assurance with Price Reporting system,GF services accessibility according to demand &social expectation,as priorities in less health infrastructure countries.GF Secretariat could give specific priority to produce affordable generic version of ART with effective price reporting system for affected population in developing countries by providing patent provision. GF Secretariat could developspecific mechanism for consideration of specific country's patent listing for drug molecule ,according to need of specific country& affected population.In developing countries ,specific drug molecule patent,price reporting system (enlist all international drugs with competitive pricing for quote a suitable supply price in developing&less GDP countries)through civil distribution channel could be helpful in easy accessibility of number of essential drugs & services for affected population. Discussion, accessibility of ART could determine priority for CSS. ---------------------------------------------------------------------

CCM structure, its working, tentative considerations, negative outcomes are all going to be part of CCM functioning cause CCM's are being driven by bureaucrats with conviction that all govt departments can function with considerable impunity, always suspecting non govt agencies of malpractices, on the contrary themselves indulging in the same. There were reports of auditor general of India where certain funds were misused by govt players ,an enquiry was initiated into it, but without any reports being made public, finding some scapegoats ,ritual sacrificing, some rolling of heads and its all done and the issue is clinched finally
Now what GFATM has to do in this context it for the Secretariat to decide and act but if secretariat considers it fair then what ‘we the people’ can do I think
Why Govt based CCM's are considered to be holy cows?

The CCM secretariat should
be a direct replica of the GF Secretariat. In order to ensure independence and
transparency in ALL its operations it needs not be linked to or depend on any
of the consistencies that makes up the CCM for financial support. The CCM
Secretariat needs to be fully financed under a regulated budget in order to
ensure that it does not depend on the PR or other local donors to survive. In
countries where the rent, office consumables and staff salaries are paid by any
of the constituencies of the CCM makes the secretariat to some extent partial
and therefore compromises on independency and autonomy. I do not deny the
significant role of government in curbing or combating HIV/AIDS, TB and Malaria
but this need to be done in the framework of transparency, accountability, effectiveness
and efficiency without any constituency considering the other as an opportunist.

Received above issue.The inclusive structure of CCM could ensure specific functioning of Secretariat in the form of PR,organisations, &MOH .The CCM could consist single national proposals,various stakeholders,PR election,LFA,linkage,national consensus,responsive to all national stakeholder,these component can give inclusive structure of CCM for transparency evaluation of definite structure of Secretariat exists in various countries& regions. For addressing various specific issues& specific implementation would depend upon various countries social expectation & GNP. A single kind of CCM model could not applicable for GF implemention addressing in various regions of world.Country social expectation, economical,climate,geo-political infrastructure could decide suitable CCM model for implementation ie.public sector,multisector,NGO/FBO, private/civil-society,UNDP. According to social priority,needs& condition,above CCM models could decide specific Secretariat selection& functioning which is accountable for addressing country's & regional products concept, financial,technical,social.geopolitical conditions& responsible for community system strengthening . I am sending discussion e-forum (CSO as a principal recipient for understanding of CCM models with Secretariat functioning.
---------------------------------------------------------------------- In selection of CSO as a principal recipient & subrecipient,CCM should consider basic GF's CSO framework (Public/private/Academic & research /Multi& bilateral organisation)&its linkage with state/ national plan,governance policies& procedures.In identification of PR& subrecipient,country CCM structure should be more comprehensive& responsive to all national stakeholders.CCM should consult with various stakeholder,LFA,linkage,CSS,& GF secretatriat in identification &selection of cicil-society organisation as a principal recipient.From GF standpoint, number of CCM models i.e. public sector/multisector/NGO/FBO/non health/private,single recipient,civil-society/UNDP/multilateral/South African,are functioning in different regions& countries,only South African CCM model consists of state participation. "Are these models constitutionally,financially,technically,scientifically& appropriately resourced & equipped in country partnership,&selection of specific civi-society as a principal recipient ." this question could give answer of CCM linkage with civil-society structure responsibilities,priorities& partnership. If GFATM(CCM) selects civi-society as a principal recipient than specific civil-society should consist of financial instruments, distribution of funding at country & state level,local NGO level, with expenditure objectives,standard allocation pattern,capacity of general equilibrium analysis with country economic& GDP growth . Civil-society should have provision of disbursal of funding according to socio-economic coverage,various sectors partnership with(trade/tourism/agriculture/industry(construction,mining pharmaceuticals,chemical,scientific& capital goods)education)could scale up CSO activity. GF could give better support to CSO in establishment of socio-economic framework,financial instrument&arrangement framework in countries. If CSO does consider inclusion of expertise enterprize resources structure than its functioning should include all components of grants recipients.

How has your CCM ensured that its secretariat is impartial, particularly where the PR, NAC, Ministry of Health or other development organizations act as the secretariat?
Normaly secretariat has good relationship with PR,NAC and ministry of health or other development organizations, always make data sharing of other institutions.
When one of the obove organizations wants to implement activities, and needs assistant easly connect the secretariat, then cooprate each other.

To ensure the impartialty of the CCM secretariat its essential that its members should also include organizations and individuals who donot have a direct stake in implementing the GF programs in the country. Like there can be representatives from the education ministry, well known social activists, media personalities, journalists, healthcare providers from both public and private sector, etc. furthermore CCM members and staff should be changed frequently and no body should be allowed to remain its member for more then 1 or 2 years. There should be no second terms.
In most developing countries many of you have noticed that the same organizations become PR and SRs again and again. Its not that there are no other implementers or applicants but because the past implementors develop a network and thus are preferred. Its like a close net group approving, deciding, implementing and evaluating the programs.
I think CCM must also have expatriates (like representatives from the local embassies) as its members and they too should be rotated. Secondly the applicants should be given the opportunity to directly give soft copies of their EOIs and proposals to GFATM headquarters and this should be properly advertised.
But its not that at all levels and in every GFATM program it happens like this. There are many examples of very transparent selection processes but still a lot can be done to further enhance the effectiveness and impartiality of CCM.
To conclude the key is to change the staff and members frequently and include representatives from a diverse strata of the society. Do not depend on old horses. There is a huge breed of young and more enterprising individuals and organizations who if given a chance can be a better bet!

Dear friends,
Regarding CCM secretariat, it is an independent but frequent monitor closely by global funds secretariat. These CCM secretariats have to monitor and advise to RRS and sub PRs. The got implementing programs have to be got prior approval or permission from the secretariat by implementing agency. CCM secretariat have to identify the PRs & sub PRs for implementing the GF Programs it has to arrange the external evaluators for evaluate the implemented programs by the PR/SPR.
There are some disadvantages of having private or another entity as a secretariat like, they are not monitor or evaluate the program properly; they accept words and activate what the implementing agency submits. Monitoring indicators may not be fully evaluated.
CCM secretariat is an impartial act, flexible with PRs, monitor & advice the implementing agency.

As Professor Jama Abdullahi Jama, I have the pleasure to send my message to the people of Somalia, Africa and the world community this unique special event day of 1, December as the WORLD HIV/AIDS DAYs Globally to celebrate the global campiagn agianst the HIV/AIDS for this event of World HIV/AIDS day for the year 2008.
I Would like to send my blessing and prayers to those affected by HIV/AIDS globally saying the almighty Allaah to give his caring and protcet and dave them, as a notice and lessons to be learnt from these people we are to take care of ourselves as those whom are not affected by the HIV/AIDS. This is a message from the creator for the evil sins and the illegal sexuals, the only solution is to make and develop confidence in ourselves the partners to control their allowed bourdaries but not beyond that.
- Regarding the point of your question, I do belive the role and the resposibilities PRs, should be identified and descripted with detailed system from the global levels to continental, regional, national and so on, therefore, GF should have to hire consultants to conduct research on this topic areas,
- For the CCM, this must be referred as the governing body of the organ to make overall control and monitoring and evaulations of the projects throughout the identified areas, which is categoried into levels etc.(will add more later on)
- NATs and the Ministary of Health should have not direct involvement in the project implementation but should have to cordinate the projects activties and implementations as well as the monitorng of the projects, The ministry of health should act and be line ministry while the planning min. will be the Planning Min. The NATs committee should be coming from multi satkeholders and should have predifined conditions and caretaria of selection, the primiary responsibility of the NATs must be to control the policy of the Organs, The GF should have to be included and involved
- The other partial bodies should be the general secretariat of the organ, which should play the most active role by performing the day-to-day activities of the organ, The skills and knowledge of these groups should be identified priorly.
The highest level of committes should be the donor and making agreemental policy with it.

Role of the Ministries of Public Health in fighting against TB, HIV/AIDS and Malaria is crucial in long run. NGOs and Civil Society Organizations could play an important role in short term. Many NGOs particularly in developing countries are changed to private business enterprises. Therefore, ministries of health should play a key role in managing CCM secretariats and closely monitor nongovernmental PRs to ensure proper implementation of Donor Funded Programs.

Sometimes it is the Principal Recipient (PR), National AIDS Authority (NAC), Ministry of Health or other development partner who acts as the CCM secretariat.
