Re: На: Lead, empower, deliver

Dear Manizha Haitova, I congratulate you on implementing important and effective interventions to combat HIV/AIDS in Tajekistan. Our organization Integrated Health Services (IHS Pakistan) is one of Pakistan's largest health services & consultancy group. We are also working on various avenues to stop the spread of HIV in our country. I believe both of us can mutually benefit from sharing our experiences as we are both woth working with predominantly Muslim population where religion can play a very positive role in making people specially the youth understand how to protect themselves. I look forward to hear from you in order to make a strategy that is workable and sustainable in line with our peculiar social, religious and cultural aspects. Best regards, Dr. Asim Mahmood Khan, executive Director, IHS Pakistan asim@ihspakistan.com

To,
CCM-FORUM

Received reminder. Regarding various community addressing, country
leadership should be well aware of socio-economical infrastructure
in less GDPcountries. For addressing specific mechanism, correct
assessment of product concept is vital component for community
system strengthening.Below mentioned discussion is based upon
addressing issues for country leadership& empowerment.
----------------------------------------------------------------------
The GF mechanism has international health financing &initiative
framework for less socio-economical infrastructure countries.
In future scenario innovative financing component could expand
formal relationship to partner environment, less GNP& less economy
infrastructure countries by providing capacity development support
including GF specific payment mechanism based on specific country's
international liquidity.
Developing countries with less economical infrastructure could
take an advantage of GF specific payment mechanism under international monetary system with Rolling continuation channel
specification.Above mentioned GF specific payment mechanism could
ease debt conversion initiative ,funding priority, formal funding
arbitration structure for recipient&sub recipient, accessibility
of GF products by countries & evaluation.GF funding arbitration
structure could give canonical support to phase funding ,fund
raised& spent including protection of rights of contributors
countries,voluntary replenishment mechanism,procuring grant agreement
in accessibility of the country's constitution& articles, identification of possible obstacles in exchange arrangement,
providing convertible monetary measures& units, correct maladjustment
in accessibility of GF mechanism & strengthening of community
system component including grant consolidation.
The spectrum of community system strengthening could be expanded
by inclusion of specific robust data analysis for accessibility
of ART for affected population including global equity strategy
addressing gender inequalities, socialization of medicines(GF Health for everyone).
Availability of complete ART drugs, proper assessment of product mix
(GF products)with systematic monitoring & accountability could ensure
effective GF mechanism implementation in less health infrastructure
countries.
GF products formation & pooled procurement of goods according to
country's specific need could be provided by country's commercial
policy variable provision with inclusion of other sectors e.g.
trade,non-engineering ,agricultural,processed products,for country's
indigenous acceptability & originality.

Center on Mental Health and HIV/AIDS in Tajikistan.
1 of December.
The Republican Marathon «Only together will we be able to defeat HIV / AIDS!» has passed in Tajikistan. This Marathon took place in all towns and districts of the Republic and brought together many people who have expressed their desire to halt the HIV / AIDS in the world. During the week, various activities were held, concerts, performances, seminars and workshops. The final stage of Marathon has become a gala concert in the Concert Hall «Borbad».

Our organization «Center on Mental Health and HIV / AIDS»  has held three major mobile Acts under the motto «HIV / AIDS is no obstacle to friendship!»  in the Agricultural University, Pedagogical University and the Tax Institute of Tajikistan on December 1. These actions have been in partnership with the City AIDS Center, and with the newly established International public organization «Association of AIDS Service Organizations”. Technical support has shares organization ACT Central Asia. These actions have covered more than 3000 students and teachers, who participated actively, responding to questions quizzes, win various prizes. Musical groups have produced dance numbers and songs. It was a survey of 100 students. The expert City AIDS Center held information sessions on ways of HIV transmission, ways of prevention, reducing stigma and discrimination. Teachers and students were satisfied with the availability of information sessions, which were held on this day. There have been many requests for conducting regularly meetings with the students on these issues. Action held by Russian and Tajik languages. Booklets and brochures were briefly and simply describes the way of HIV transmission and ways to prevent HIV, as well as addresses and phone numbers of organizations where they can be tested and get help and support. The volunteers of our Center conducted a full day public education campaign. Our bus with color balloons, flags and banners drove in all streets of the city, attracting the attention of passers who have, can the same thought, and recalled that on December 1 - World AIDS Day .

We believe that such acts should be held not once or twice a year and regularly. It is not worth one day to concentrate its work on such activities. In the next 12 months, our organization has scheduled regular Acts in the cities of the Republic of Tajikistan. We also plan to hold a Round table with religious leaders and according the Project «Islam condemns stigma and discrimination of PLHIV» with the support of ACT Central Asia.
Our center, which is a public organization, received a License for medical activities perhaps for the first time  in Central Asia. Our organization conducts researchs, trainings and seminars with groups such as PLHIV, injecting drug users, health workers, service providers, journalists and religious leaders. In our Center conducted psychological and before test counseling and self care groups for PLHIV / IDU.  Now we plan to provide  not only psychological, social and legal counseling, but also high-quality medical service.
Perhaps for the first time in Tajikistan will be fully implemented the Law «About HIV», which governed free medical care to PLHIV. And these issues we also intend to engage in 12 months and of course continue in long future.

YOUTH HIV FORUMS at Colleges & Universities in Islamabad

Involving the Youth - Targeting the Future!

Youth are the future. They have fresh & receptive minds full of energy & zeal. Changing their perception can change the perception of the future generations. With this target Integrated Health Services (IHS Pakistan) is implementing a pilot project since June 2008 in Islamabad titled “Youth HIV Forums” with the support of World Bank. It aims to utilize the immense voluntary potential of the youth and channelizing it to reduce the stigma associated with HIV/AIDS.

In the first phase HIV Youth Forums are being established at 12 colleges & universities of Islamabad to create awareness about HIV/AIDS among students of age group 17 to 22 years. 359 students with 63% males and 37% females have become members of the Youth HIV Forums to date. The colleges and universities participating in this project are Quaid-e-Azam University, AIMS, SKANS, SZABIST, Preston University, Indus College of Commerce, Islamabad College of Management and Commerce, COMSAT, MAJU, Iqra University, Alta Vista College & Federal Urdu University.

Forum members are educated about the disease by IHS project doctors regarding its spread and why is it stigmatized. Forum members then take up activities to specially highlight that it’s not just a sexually transmitted disease but can be transmitted through other means as well just like Hepatitis B & C and that the HIV patients are not untouchables. To increase the case detection rate (CDR) it is important to reduce stigma associated with AIDS patients for which it is necessary that other modes of transmission of HIV like reuse of syringes, unsterilized surgical and barber instruments, etc. must be highlighted instead of the common perception of linkage of HIV with sex. The student members of the Youth HIV Forums are involved in activities like creating awareness among their peers, friends, family & community, holding theatres and competitions in their colleges like painting with themes like “HIV Patients are not untouchables”, “AIDS is not a Stigma” or “Fight the Disease not the Patient”. Members also visit hospitals to provide counseling to HIV/AIDS patients & their families.

HIV Forums can become a part of the traditions & culture of educational institutions just like ‘Science & other Clubs’ in schools and colleges so that they sustain themselves and continuously instill a sense of voluntary activity in the minds of students. They can easily be sustained by the colleges once the tradition is set and can also be replicated at other educational institutions. We just have to introduce the concept and then it will go on. IHS Pakistan on similar lines is successfully implementing a GFTAM Round-6 funded project to make “TB Clubs” in schools and making children the ‘Ambassadors of Health’ in their community. In the last one year across the country in rural areas of 10 districts including Quetta, Tando Allahyar, Tando Muhammad Khan, Gujrat, Mandi Bahauddin, Khushab, Sialkot, Narowal, Muzafarabad & Bagh the School TB Project has educated 25,000 school children and from among them 1500 have become Ambassadors of Health who are now actively creating awareness about TB and other communicable diseases in their communities.

Sensitizing the youth is a positive step to reduce the spread of HIV as youth have just entered sexually active life and this is the right time to tell them in clear words that how they can and they should protect themselves. A pre-intervention survey was carried out which will be followed by a post-intervention survey in June 2009 to assess the effectiveness of the Youth HIV Forums. This is the first of its kind intervention being carried out in the world where specific forums of youth to fight HIV/AIDS are being made in the educational institutions. It has a potential to be replicated in order to stop the spread of AIDS especially among the youth. It’s important to note that during the last 5 months of this project 11 undetected cases of HIV have been identified in Islamabad by the youth through motivating people to get screened for HIV. The identity of positive cases has been kept secret however they are being followed up by forum members to ensure that they seek treatment and are educated to curtail the further spread of the disease.

Dear Javed S. Ahmad and rest of the world,

I am agree with you and also serious about treatment literacy on HIV and AIDS. May be this is the time to reduce misinformation cost on it around the world.

I do believe that communication and proper communication only can help people meaningfully and miscommunication can do reverse doubly.

Anyway, I am trying to calculate cost of misinformation and committed to deliver analysis by next year.

Re: Lead, empower, deliver

Dear Collegues,

HIV/AIDS, despite two decades of global action, is still rampant. Even in USA, the numbers of infected persons is rising. Unfortunately, I am not convinced that the leadership at the top of the organizations engaged in fighting the curse, is all too concerned with prevention as they are with the treatment. Most money and effort is devoted to diaognosis and drug therapy, because that is the way expenses are distributed. Sometimes one may even suspect that the entire anti-HIV/AIDS program is set up to support pharmaceutical industry.  I am sure that is not the case.

So what is my personal commitment? As a Communication Consultant, I will continue to speak, at every forum, on the importance of prevention of HIV/AIDS, particularly by better communication strategies, that ensure bettter understanding among the population at risk, of how the disease is transmitted and how it can be prevented, and does not admonish people to use one method of prevention or the other (e.g., A, B, or C) to protect themselves. Communicator's job should be to present all the options and let their audience make the choice.

I will keep pushing for this approach until I am convinced that my approach is wrong or I become totally ineffective in convincing others. Regards.

Re: Lead, empower, deliver

Dear friends,

Greetings to those who voluntarily participate and organize the 20th world AIDS Day on 1st December 2008.

Mother Therasa Trust organized a rally with 200 men, women and children at Genguvarpatty town, where the migrants, contracted construction workers are predominately. The rally imparts about the ‘ABC’ concept, behaviour changes and safer sexual activities. The slogans and hand hoardings are much imparted about the safer sexual, communication on behaviour changes.

Mother Therasa Trust (MTT) serves in gross root level among our target population commercial sex workers, industrial labourers, migrants, drug users, Truck drivers to prevent and control measures of HIV/AIDS/STD.

We planned to generate awareness of HIV/AIDS/STD among the target groups. We have to train the peer educators among the risk behaviour groups. MTT prefers to make referral cases to ICTs and STD clinics.

v     MTT would like to undertake care and support programs through supply of ARV drugs, Nevirapine etc.,

v     MTT planned to provide skill development to the family members of the PLWHA.

v     MTT have to arrange financial assistance to generate income to the family who are infected with HIV/AIDS.

v     MTT have to arrange the social equality within the community of those infected HIV/AIDS.

v     MTT planned to propaganda to rural illiterate population about HIV, ways of transmission of HIV and preventing and curing methodologies of the HIV/AIDS/STDs.

This is the MTT’s future plan for the next 12 months.

Thanking you,

Yours,

Re: Lead, empower, deliver

To,
CCM FORUM

Received above issue.Regarding community addressing, country leadership&empowerment, understanding of social& structural opportunity
are essential for various community participation in developing
Asian& African countries.Country leadership& empowerment could
be answer of discrimination in social conditions& vulnerability
of various diseases in developing world.
Discusion e-Forum 2008(strategies& policies)could include community
system strengthening, country leadership for addressing strategic
intervention, policies,& empowerment evaluation for gender concept.
----------------------------------------------------------------------

Received final discussion. Regarding global equity strategy for
addressing gender inequalities in country program , GF mechanism
has CSS with gender, affected population, sexual minority components
for specific addressing. Socialization of medicine is vital for
social gender equity.Accessibility of ART( 25(ART)drugs) is
essential for health system strengthening in less health infrastructure
countries. In developing & under developed countries gender sensitive
approaches for sexual minority(women & girls) could give additional
capacity building for empowerment, social equity, universal coverage,
gender socio-economic indicators i.e. specific gender labour,
natural resources, calorie availability & social institutions.
GF mechanism should focus on gender behaviour data, universal coverage,
HIV statistical information in affected countries, ready access of treatment in less health system infrastructure countries,
social intervention & accessibility of country's constitution for
prevention of diseases through GF products& services. I would
like to give reference of Gender theme for gender sensitive approach,& effective implementation of GF strategy.
----------------------------------------------------------------------
Received theme 1: gender

Regarding gender sensitive approach & responsiveness all three
mechanisms are important for strong & more effective partnership
for sustained impact.
In gender sensitive program planning new gender aspect can include
improved overall nutrition& bio-availability for lactating
mothers & undernourished,under weight female children's.
Understanding of genetic factors , differential resistance to
diseases, prevention of gender sensitive cultural& traditional
practices.
Business models, TRP,can also help in elimination of mal-practices
of gender sensitive approach.Business models should be related to
improving socio-economic condition of marginalised ,gender condition
vulnerable to climatic condition,socio-legal rights of infrastructure
natural& productive resources, civil society participation with
rural community focussing on gender sensitive approach.
Business models should ensure better access to markets, improved
working& service condition, legal aid in the unorganised business
sector& basic services, literacy level, adult education, universal
primary education.
TRP& Board are technical concepts which are very useful in prevention
strategies& treatment ,awareness, behavioural change ,capacity
building & health care provision in gender sensitive approaches
& responsiveness.
Other mechanism can include humanitarian approaches with administration support, organisation that are gender sensitive may
catalyse& encourage gender responsiveness, country leader-
ship, basic services education, health, financial infrastructure
& universal benefit.

Re: Lead, empower, deliver

I commit myself to begin the fight from within my own family by educating them on the disease itself, mode of transmission, life style changes and behavior that will lead to responsible sexual habits and for those who can not  do without having sex should use condoms properly and fidelity tfor those that are married.I will also encourage them to talk about HIV/AIDS with their friends both at school and back in the  community and to form peer educator's group that will help them know more about the disease and the consequences on the socio-economic and political aspect on the country as a whole.
For those infected and have support groups existing,i will like the government of my country to empower them with capacity building activities like making soap, rubbing oil and detergent production that will generate income into the group. This will enable them to take care of their children and also feed themselves considering the global food crisis. In line with the global food crisis, the government should look for means of making special feed formula that will contaain all the essential nutrients for them.
Antiretrovirals should completely be free for those living with the disease.some of the patients  end up dying at home because of the lack transportation means and os if possible their fare to the treatment centers be subsidized. As a solution, more treatment centers  should be open and equiped with trained personnel and equipments at all district hospitals for acessibility to all.If the above measures are put in place,i believe many people from the community will come out to know their serologic status and funds destined for  the community should the locality without any administrative bottleneck huddles because this is where the progress of the program is usually impeded.
With this we are sure of quality health care to all, improve life style and positive behavior changes as hiv/aid is concerned.

Re: Lead, empower, deliver

I intend to conduct rural community outreaches, albeit on a small scale, because sad to say, I think there is still too much HIV/AIDS and antiretroviral ignorance.    I am proud to have encouraged, two female friends who were terrified and averse to testing, to brave their fears and got tested by merely stressing that a negative result meant ‘no more nervous tension’, and a positive meant, ‘medicines for the stamina to enjoy the world and care for the beloved’.   Within a week of our discussion they got tested and live happily ever after. Humanity’s curious psyche is more inclined to react to the beneficial facts. I doubt that 40% of our population knows the meaning of antiretrovirals, perhaps the ordinary, illiterate person need to hear ‘medicines for’.

I have led ‘red ribbon’ campaigns where we affixed ribbons to street lamp poles stretching 12 miles up to a community college; we visited the nearby offices including two radio and one TV station, and pinned a miniature red ribbon on the staff; we pinned ribbons on almost every person we encountered.  The college students flooded us for information leaflets.  The educational event culminated in a grand finale with a gathering of co-workers and invitees.     Many students and adults enquired the meaning of the red loop.  Another year, I organized a marvelous event for staff where representative of each island placed a natural red rose in a vase, forming a beautiful bouquet which we surrounded with lit candles, while the CEO of the organisation released a bunch of 90% red, 10% white balloons.   The media was present and reported on the events.

My newspaper article ‘AIDS We’re Not Doing Enough’ in 2005, right after World Aids Day, generated much radio discussions, and thus I hope in 2009 to write another such provocative topic.  Writing this blog has enticed me to aim for a bigger community outreach involving nurses and sports club youth members in a house-to-house campaign.
